Award Description: Granted to Sworn or Civilian Department members on active-duty status between May 18, 2012 and May 21, 2012 that provided service to the City of Chicago during the NATO Summit.
Presentation: This award is presented to Sworn Department Members in the form of a ribbon bar and certificate. Civilian Department Members are presented this award in the form of a certificate.
